An annual health checkup is a more extensive visit in which a comprehensive physical, mental, and developmental evaluation is done to make sure your child is healthy from head to toe. In addition most children will get a vision and hearing test and a review of daily routines to make sure healthy guidelines are followed for nutrition, sleep, exercise, and school performance.
New Patients:
Having your child’s prior medical records, vaccination records, lab results, and any other relevant personal or family history will allow for a more smooth transition to Park Pediatrics. If your child is having difficulty at school; bringing a report card and any written documentation or testing by the school will help us tremendously.
Don’t Hesitate to Ask Questions:
Come prepared to discuss any concerns you may have about your child. The pediatricians at our Takoma Park, Greenbelt and Gaithersburg locations are always happy to answer your questions.
School/ Sports Forms:
If there are any school or sports forms that may need to be completed, please bring these forms to your childs annual check-up. Please let the receptionist know at the start of the visit that these forms will need to be completed by the provider. We need one week to complete the forms and return to you; so please prepare accordingly.
