Park Pediatrics is currently accepting new patients at all of our locations: Takoma Park, Gaithersburg, and Greenbelt. Please note we do not accept non-vaccinating families. Please review the information below regarding how to become a patient at Park Pediatrics:

Please call our office to set up your child’s first appointment. We will ask that you notify your child’s health insurance that Park Pediatrics will be the new ‘PCP’ (primary care provider)- effective immediately. Depending on the insurance company, we will guide you on who to select for the new PCP. For example, it may be sufficient to state ‘Park Pediatrics’; however for some insurances you may need to choose a certain pediatrician in the practice such as ‘Dr. Lavanya Sithanandam’ or ‘Dr. Louise Wong’ specifically. Please bring the following to your child’s first appointment: insurance card, any prior medical/vaccination records (including international records), knowledge of prior ER or hospitalizations, knowledge of family medical history, and any school or outside forms that need to be completed. (Please remember that it may take us up to one week to complete such paper work). If your child is being seen for school issues, please bring any forms from the school such as a copy of the IEP (Individualized education plan), report cards, or other communication with the teacher. Please download and complete our age dependant developmental forms and questionnaires for your child’s check-up and bring them to the appointment.
